Services
There are often times an individual requires a more creative option than what is currently offered. Not every individual fits into every environment. We specialize in out-of-the-box solutions to meet the needs of the individual.
Creative Options, LLC currently serves individuals receiving funding from the Level One, SELF and Individual Options Waivers.
Creative Options is certified by the Ohio Department of Disability to provide the following services:

*Non-medical Transportation
*Homemaker/Personal Care (we are not currently taking new referrals)
*Supported Employment Community (we are not currently taking new referrals)
*Supported Employment Enclave (we are not currently taking new referrals)
*Supported Living (we are not currently taking new referrals)
*Community Respite (we are not currently taking new referrals)
*Residential Respite (we are not currently taking new referrals)
*HPC Transportation (we are not currently taking new referrals)
Although we have been approved for all of the services listed above, we may not be providing services at this time. Please contact us to see how we are able to meet your needs.
Adult Day Support
Creative Options ADS operates Monday through Friday, from 9:30 AM to 2:30 PM. Non-Medical Transportation (NMT) is available to participants of both programs within a 10–12 mile radius of the site.
Onsite (Campus) Programming:
The Onsite (Campus) Program focuses on building and enhancing participants’ developmental and social
skills to promote greater independence. Programming takes place primarily at the Creative Options campus,
with 2–3 community outings each week.
Typical outings last 60–90 minutes, while extended, all-day outings (lasting 3–4 hours) occur once or twice a
month. We emphasize small group settings to provide personalized support and attention, maintaining ratios of
no more than 1:5, though most groups are closer to 1:3 or 1:4.

• Math & Money Skills
• Creative Writing Classes & Book Club
• Language: ASL, Spanish
• Fitness: Dance, Zumba, Yoga, Valemee Fitness
• Music Therapy
• Cooking Class
Examples of Weekly Outings:
• Bowling
• Shopping (Dollar Tree, Five Below, etc.)
• Library Visits
• Park Explorations
• All-Day Outings may include: the Zoo, fairs, museums, movies, etc.
Community Inclusion (Integration) Programming:

The Community Inclusion Program is designed to help individuals explore career interests, develop job-
related and volunteer skills, and increase comfort and confidence within the community. Participants engage
directly with local businesses and organizations to gain hands-on experience and discover their strengths and
preferences.
This program operates in the community for at least five hours per day and maintains small group ratios of
no more than 1:4, ensuring individualized guidance and meaningful engagement.
